Office Policies
Appointments
Patients experiencing moderate to severe acute symptoms will be scheduled within 24 hours.
Patients experiencing mild or chronic symptoms will be scheduled within 1 week.
Patients in need of a well visit or physical exam will be scheduled for the next available business day, which may range from within a week to within a month depending on provider/location preference or current volume of appointments.
All appointments missed or cancelled less than 24 hours in advance will be assessed a fee of $25.00.
Payments
We will gladly accept your payment by cash, Visa, Master Card, Discover, or American Express. Personal and travelers’ checks are not accepted. All co-pays, coinsurance, and deductibles are due at the time services are rendered. You will be notified at check-out if a deductible and/or co-insurance will apply. It is your responsibility to stop by the check-out desk to pay for the services rendered. If payment is not received at the time of service, a $30.00 late fee will be assessed for every statement mailed. Any unpaid balances are to be paid in full unless prior arrangements have been made.
Newborns
Newborns must be added to your insurance policy from birth. On your first newborn appointment you will be required to pay a $100.00 non-refundable deposit for any newborn patient not active in the insurance at the time of service. Any subsequent visits will be charged as a $200.00 non-refundable deposit for any newborn patient not active in the insurance at the time of service, excluding vaccines as they vary per patient need. Please understand that this charge is non-refundable. Per our office policy, we cannot resubmit claims for newborn visits, even if insurance becomes active retroactively since the patient’s date of birth.
Circumcisions are an elective procedure not covered by insurance, but available as an out-of-the-pocket service for a $500.00 non-refundable deposit. It is your responsibility to keep us informed of any issues you may encounter with this process.
Accessibility
Parents with language needs can schedule appointments with providers who speak Spanish and Portuguese.
Children with special needs (such as autism, ADHD, or physical disabilities) will be cared for appropriately by providers trained to accommodate their specific needs.
